Threshold House was founded by a group of death doulas who saw a gap in Philadelphia that no one seemed to be filling. The team has rapidly expanded to include hospice physicians, chaplains, hospice social workers, nurses, and community advocates.
Some of us have sat with people in the last weeks of their lives and watched what happens when a terminal diagnosis arrives and there's no caregiver, no safe place to stay, no one able to carry the weight of what comes next. Others among us have spent years doing deathcare work throughout Philadelphia — accompanying people and families through dying, death, and grief, in homes, hospitals, and hospice settings across the city.
We kept encountering the same story: someone dying who simply had nowhere to do it. Not because hospice wasn't available, and not because care wasn't wanted — but because a safe, supported place to receive that care didn't exist for them.
Threshold House is our answer to that gap. We are building a home in Philadelphia where no one has to face the end of life without a roof, without support, or without someone beside them.

Following in the footsteps of the Omega Home Network national model, Threshold House will provide more than care; it will be a place for loving support and presence. We walk with you through fear, pain, uncertainty, and the practical and emotional weight of dying, being willing to lean in and listen.
We are not a hospice, and we don't provide medical care. We will work alongside hospice providers, offering what our training and years of experience have showed us matters most in someone's final days — steady, non-medical,human support, for as long as it's needed.
